About Our Program
Family Health Centers of San Diego PACE (FHCSD PACE) provides individualized care by an interdisciplinary team of professionals focused on working with you to establish goals for care and address specific needs so you or your loved one can continue to live safely at home. As we age, our health needs can become overwhelming. PACE helps to manage those needs through coordinated medical and social services. If your goal is for you or your loved one to stay at home, FHCSD PACE may be the right choice for you!
What We Do
PACE stands for Program of All-Inclusive Care for the Elderly. PACE is a Medicare and Medi-Cal program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.
Why We're Different
FHCSD PACE is an alternative to nursing facilities for people 55 and over that provides customized health care and social engagement to older adults to help them stay independent and in their homes. PACE not only offers medical care, but also transportation to and from appointments, meals on-site or delivered to the home, physical and occupational therapy, home care and a place for older adults to enjoy recreational activities.
