FHCSD PACE - Primary and Specialty Care

We offer primary and specialty services by medical professionals trained in older adult care.

Your plan of care is personalized with the assistance of your interdisciplinary team: primary care provider, registered nurse, recreation therapist, social workers, PACE center manager, transportation manager, personal care attendant, home care coordinator, dietitians, physical therapist and occupational therapist. Together, they develop a plan and deliver all health care services as needed.

We offer the following medical and non-medical services to our PACE participants:
