Frequently Asked Questions

General

Family Health Centers of San Diego (FHCSD) PACE provides individualized care by a team of professionals focused on addressing the social, physical and mental health needs of older adults so they can continue to live safely at home.

PACE participants have access to a variety of coordinated services, including:

 

  • Primary and specialty care – medical professionals who help coordinate your health care
  • Transportation – to and from the PACE center and external specialty appointments approved by the interdisciplinary team
  • Vision, dental and audiology – eye glasses, dentures and hearing aids
  • In-home care – assistance with personal care and light housekeeping
  • Medication assistance – prescription medication delivered to your home at no-cost
  • Social activities – recreational therapy programs such as music, art, exercise and more

Cost & Eligibility

PACE is a health plan that takes the place of Medi-Cal, Medicare or both, depending on eligibility status. As a program participant, you agree to accept all services from FHCSD PACE and its contracted providers.

You are eligible to join FHCSD PACE if you:

 

  • Are 55 years or older
  • Live in a designated PACE service area
  • Are able to live safely in the community
  • Meet the level of care requirements as determined by the California Department of Health Care Services
