If you have used 911 emergency services or have been admitted to the ER, please notify FHCSD PACE as soon as possible. 

Your assigned PACE primary care provider (PCP) will inform the emergency service providers of your medical history and work with them to establish your follow-up care and treatment plan, if necessary.
